i think it would depend on how long you have been dating. if it is a first date she might want an easy out. or perhaps she wants to meet somewhere inbetween where you both live. i wouldn't read too much into and just enjoy your date :)

No it means she doesn't know you very well and she is being smart. Don't take it personally and do as she request no questions asked.
If she wants to meet on the thrid or fourth date drop her.

Or she's got a boyfriend/husband at home she doesn't want you to know about... or her house is a mess ... or her mom and dad are weird... or she lives in a box under a bridge, and doesn't want you to know... or her neighborhood isn't safe...
