Give one sheet with your name, the name of the university, your current GPA at school, the list of other activities that you do at school (extra-curricular, they might not know), and why you chose that university.
The more information you give a reference, the better the letter will be. Be SURE to put the deadline for sending the letter, and always follow up with a thank you note (handwritten, and not email).
