Yes, you should give them a stamped, addressed envelope (type the address) but leave the return address area blank. Give one sheet with your name, the name of the university, your current GPA at school, the list of other activities that you do at school (extra-curricular, they might not know), and why you chose that university.

The more information you give a reference, the better the letter will be. Be SURE to put the deadline for sending the letter, and always follow up with a thank you note (handwritten, and not email).

Give them the form to fill out, and the stamped and adressed envelope. Some teachers will like to talk to you, offer to do that, and maybe write them a note telling them what your college goals are and thanking them.

A stamped envelope would be good so they wouldn't have to pay anything to send your letter. I would also give the professor a resume of yours just so they know everything you're involved in.
