1) Pliny the Elder died in the eruption, on a boat off the coast, while trying to reach the survivors of Herculaneum and Pompeii. There were very few survivors if any in Herculaneum. In Pompeii only those people survived who never went back to get their chattels after the first ash falls and before the mud flows. The story of the Vesuvius eruption and the end of pliny the Elder is told by his nephew, Pliny the Younger, stationed at the Roman Naval Base 20 miles away in Cuma. Interestingly the US Naval Station of today is not far from the ruins of Cuma.
2) A 'Plinian' eruption (a big one) of Mt. Vesuvius is overdue. The Italian Volcanological Service is actively monitoring the volcano and expects up to several days warning. Evacuation plans have been devised and (somewhat) tested, but only the will of God will ultimately decide how well they will work. Even so we might have to wait tens of years before it happens.
3) Naples is a very densely populated city and the pressure for living space is high; the land around Mt. Vesuvius is very fertile, as lava is full of minerals and plant nutrients. To prevent the continuous expansion of the city to close to the danger zone, as well as to preserve and study a unique habitat (while it lasts), the Mt. Vesuvius National Park was instituted.

They went to Naples, but as Herculanium was taken out by a lahore and not the eruption itself there weren't may escapees there and Pompeii people stayed because they believed they were safe ubtil it was too late and they died choking in the ash as they tried to escape. Those that did escape went into the countryside and eventually made it across the bay to Naples after the eruption.

there were few if any survivors but the city of Naples was nearby and Herculaneum is within the resent day city limits
