Originally the first world countries were the U.S.and Canada and western Europe. The second world was the Soviet Union (now Russia) and its satellites and allies. The third world was the remainder, unaligned and in general poorly developed countries.
The third world often benefited from the competition between the first and second world. Often they did not as the U.S. and its allies fought proxie wars with the U.S.S.R. in various countries (Viet Nam and Nicaragua are 2 well known examples but the list is long).

The 3rd world countries continue to be poorer and are often called less developed or developing countries. Poverty and ignorance and combine to cause high birth rates which in turn drives poverty and ignorance. The cycle is hard to break, obviously, but it is possible, and has been done.

To qualify for being a less developed country you need to be low on health care, employment, education, and so forth. AN unstable political history also counts but the form of government doesn't matter, all have proved incompetent someplace, some time.

Other factors are relivent and do occur, but that is the basics.
