You didn't say which area, but I would suggest starting with your state's Department of Rehabilitative Services and your local Social Services Dept.
You can also contact local American Legions, Jacyees or Civitan groups. They sometimes provide donations towards projects like this and sometimes will even take that on as a project.

Depending upon what state you live in, there should be programs that are state and/or federally funded. Here in Texas, you daughter would probably qualify for the CLASS program (Community Living and Supported Services). To qualify for CLASS, you (or your child) must have a primary physical diagnosis (like Cerebral Palsy, Autism, Multiple Sclerosis) and the diagnosis must have been made prior to the age of 21. We also have another program called HCS (Home and Community Based Services) but you must have a diagnosis of Mental Retardation to qualify and right now, we have a 10 year waiting list for that.
Check the website for your states' offices of Mental Health and Mental Retardation or your states' Department of Health and Human Services. Your state's agency might also be something like "Department of Aging and Developmental Services".

This is a hard question to answer without knowing what country you live in. You already have some answers based on the USA. If you live in Ontario, Canada, the Assistive Devices Program will pay a significant portion of the cost. I assume other provinces have similar programs. A phone call to your provincial or state government should help you find out what is available where you live.

Call the Department of Developmental disablities, they may have funds or could help you fid a source. Medicaid also has some funds for home modifications.

Some times Habitat for Humanities will help. Call them. Also, talk with some friends at your church and see if they can get something started. Usually there are enough talented men in the church to offer free labor.
