Because she wants the spa experience, and would like to spend that time with you. It's really no big deal and you'd be really cool for manning up and doing it. Maybe she really thinks it's time for you to get a touch up!

a massage is okay but facial, pedicure and manicures are girly for a guy to get but some metrosexual guys are into that kind of treatment and go regularly to get themselves tuned up.I wouldn't subject my boyfriend to that kind of torture and shame.well, if you decide to go, don't let your guy friends know about it or else they might tease you for the rest of your life.if she wants a memorable couple experience, maybe suggest that you go minature golfing or bowling or somewhere that the both of you enjoy.unless you don't mind and she really wants you to go to the salon/already made arrangements and you would like to at least try it to make her happy, then go for it.

It's her birthday & she wants you to have some fun with her, big deal, it's just a salon, you're getting a makeover, you not going to walk in her boyfriend & leave as her girlfriend, so just get your facial, don't forget to tell them to use clear nail polish when you get your manicure and pedicure.

if u want to do this ask him (saloon) to do herbal facial. it is very good for ur face. and first know that what type of skin u have. then go for facial. if u dont want to go to saloon u can do facial in home itself. in every shops u can get facials mask packs, direction will be given in that pack. u can apply that in home and leave for 15 minutes and rinse ur face with cold water (without using soap). don't use soap for atleast 4 hours, it may harm ur face.
and for manicure and pedicure take a tub(small) of water add some shampoo and cooking soda in that then soak ur hands in that for 5-10 mins then rub ur hands with half lemon and rinse with that same water and if u need rub ur hands smoothly with fumic stone (this will be available in all fancy stores.) it will removes dead skin. and rinse with good cold water. then finally apply any good moisturiser to ur both hands. do same thing on legs also. if u go to saloon they also do the same and they will charge this for hardly 500 rupees

of course you should do it! it's her birthday, you should do anything to make her happy, especially on that day.
and this could be a one-time opportunity, who knows? maybe you'll have a blast! i loved it, it's soo relaxing.
my uncle went to a spa for a weekend once haha. he said it was the nicest feeling ever.

there is perfectly nothing wrong in doing it , infact it is a very good sense of hygiene, that she is trying to install in you, take it this way - mentally it will benefit you, and plus it adds to your appearance , boosting your personality.
trust me it is a very good suggetion, and nice birthday gift.
she will also feel confident when walking with you
