In the living cell, DNA undergoes frequent chemical change, especially when it is being replicated (in S phase of the eukaryotic cell cycle). Most of these changes are quickly repaired. Those that are not result in a mutation. Thus, mutation is a failure of DNA repair.
There are many types of mutations, example MISSENSE MUTATIONS, Nonsense mutations, Silent mutations, and,Splice-site mutations.

Do Useful Mutations Occur? -- If the plant or animal lives better then I would say yes they do. Being human how would we know what mutation took effect? Maybe the animal hears or sees better, but looks the same. Or a plant gets more energy with less sun - how would know to measure this? It sounds like that if animal and plants don't walk or talk to us or build a rocket there is no evidence of mutation. Probably because humans only live such a short time - in 1 million years I'm sure we will notice a difference in Elsie the cow, as she drives down the road to get her nails done at the mall.

Mutation is the alteration of DNA sequence, whether it be in a small way by the alteration of a single base pair, or whether it be a gross event such as the gain or loss of an entire chromosome. It may be caused through the action of damaging chemicals, or radiation, or through the errors inherent in the DNA replication and repair reactions. One consequence may be genetic disease. However, although in the short term mutation may seem to be a BAD THING, in the long term it is essential to our existence. Without mutation there could be no change and without change life cannot evolve. If it had not been for mutation the world would still be covered in primeval slime!
It can be heritable or inheritable. while heritable mutation are beleived to be sex linked and thus passes from one generation to another, inheritable is not and may soon wipe out in the ecosystem

Mutations occur because of many reasons: chemicals, exposure to Ultraviolet Rays, Radiation and sometimes spontaneously. Many mutations occur during cell replication/cell division, and can cause irreversible disadvantageous affects.

ok when cells are going through the phase called Anaphase the chromosomes are beeing pulled apart and there should be an even number and the right kind of chromosome on each side and if one of the wrong chromosomes get on the wrong side....Ex. 2 red chomosomes both get on the right side).That causes a mutation to acure.There are also some other ways a mutation can acure but this seems to be the biggest reason this happens.

When cells do not copy themselves exactly.
This can be an error during mitosis or meiosis.
The DNA strands are not copied exactly from mother cell to daughter cell so a mutation occurs.
