YES. You can get a copy of all your records transferred to your new doctor. It can happen two ways; (1) ask for a transfer of records request from your present doctors office, just ask the receptionist, fill it out with the new doctors info, your social security number, date of birth, etc. Be sure to note when your new doctor's appointment is scheduled, giving them no excuse not to have it sent in time. (2) have your new doctors office send the request to your old doctor, you'll fill out the same sort of request, yet your new doctor will have it forwarded to them. As you can see, it's the same process. No need to panic when it's not there for your new appointment. Also, if you need hospital records, go to their medical records department and do the same process there. They should hand them over or send them over. Take care of yourself.

As a nursing student, I know that a patient is allow to have any copies of any file that is in their record at any time. However, if the original documents are being requested this may be a more of a complicated issue. As long as its a copy then you should be good to go. You should also try to find out if they can send your record to you new doctor. That may be alot easier too. I hope this answer helped in any way.

Yes, but they usually wont just make a copy for you and give it to you to take with, but when you go to the new doctor, you willl have to sign a medical release form and they will fax it to your old doctor, and in turn they will fax your records back over.

Absolutely. All records actually belong to you. A doctor, just like a plumber is someone that you employ. Feel free to fire one at any time. Be prepared to pay them for making a copy of your record (usually a nominal fee) and you probably will not get x-ray films unless you make it clear that you are leaving their care permanently.

Yes, you can call your doctor and tell them you want a copy of your file or you can sign a paper at your new doctors giving them permission to have your file faxed over. Sometimes if you ask for a copy of your file they do it for free and sometimes they charge you. Signing the paper at your new doctor's for them to have your files faxed over is free.

Your medical records belong to YOU not the doctor. He or she will forward them directly to the new doctor at your request.

In Canada, yes, although you'll probably have to pay for the photocopying (you don't get the originals, they stay with the doctor).
