I think they are saying that because Barack Obama is related to Dick Cheney, that he would support torture.
I didn't know they were related, to begin with. But I guess it's kind of funny.

Look at the newspaper the woman is holding: "Dick Cheney is related to Barack Obama". By saying the child who wants to be a Medieval Torturer takes after his side of the family, she is implying Dick Cheney is a Medieval Torturer.

Basically it is a backhanded comment on Dick Chenys opinions and allowences of torture. The magazine is reminding us of a recent claim that Cheny and Obama may be distantly related. The midevil torture is reminding us of Cheny and the fancy waterboards and other such tortures used in interogations of terrorists.
In short it is using Obama to make fun of Cheny.

It's a reference to Obama's muslim heritage on his father's side. Islam is often crticized for not having gone through a reformation period, like christianity did, in which it was examined and liberalized, thus leaving it "in the middle ages."
The cartoon is probably intentionally oblique, since it's not a politically correct joke, at all. Nor, of course, is it in any way relevant to Obama's candidacy.

You have to read the headline on the newspaper his wife is holding that talks about Cheney and Obama sharing an ancestor. It is saying that anyone related to Cheney must have torturing in their genes.

It is saying that since the kid wants to be a mideval tourterer, he got the idea from his mom, not barac obama who everyone thinks is a terrorist because of his name sounds like it and he was raised in the middle east.
