That that person was rude. No matter what if he had a problem with his hand and couldn't shake it, then he should have mentioned it. Otherwise that is just bad manners. I always reach out my hand to shake it when I meet someone, I think that contact and speaking make such a difference in how we are perceived.

I'd be a little put off, but I'd get over it.. I hate it when casual introductions amongst a large group of people are met with handshakes.. it just gets awkward.
If I'm at a business lunch and being introduced to executive members or something, sure - but if I'm at a halloween party being casually introduced to people I don't know, I don't feel like giving 50 handshakes - especially when most of the girls that were sticking their hands out to me were offering me a limp wrist.
If you're going to shake, shake with gusto - if you're going to hand me a clammy, floppy hand, don't bother.

I'd be offended. The person would obviously be some weirdo with OCD who is afraid of germs. "He think he's better than me and that I'm unclean." That's what I'd think.
