It will be me, my fiance, and 2 children. I am the only one who filled out the rental ap, and it is solely in my name. Am I the only "tenant"? I am trying to fugure out if I can go ahead and fax this back to them or if my fiance needs to sign too under "tentant signatures".

Until your fiancee's name appears on the lease, you are the only tenant of record (and in some places, having a non-tenant living with your - him, not the kids - could result in the leasor initiating eviction proceedings). Let the Rental Agency (or Building Owner) that when the lease comes up - if you plan to stay - you would like to have his name added to the lease.
Right now, YOUR credit rating is the only one at risk, so be careful about how things go if you break it off with him (assuming the worst)...If things go well, once his name is on the lease, you are both protected, since each of you will have a vested interest in maintaining the appartment.

As for the Renter's/Tenant's Insurance...talk to the agent about that, and indicate that you would will be adding the fiancee to the lease at the first opportunity. If that is only a few months away, obtain a temporary policy that will expire at the same time as the lease, then buy a new policy with both your names on it.

You are going to have to discuss with the landlord/management. Many require all adults over the age of 18 turn in an application and go through the credit/criminal/background checks.

Let me say, I'm not a lawyer, you may want to check with one. If you are the only one listed, you may get in trouble for having other people there. But if you don't list others, you can have the others thrown out if you want.
But if you list all, you have equally rights to be there.

what was the conversation you had with the landlord? some landlord do not care about extra people some do and would considered the fiance living in the unit to be a violation

It doesn't really matter. You can leave it the way it is, and you can add the other person later, if the management asks you to do it. Of course, if you add another person, you'll make him officially responsible for the rent. If this is something you worry about, then do it right away.
