Are you asking about getting a fraudulent one? I wouldn't know that. But if you mean a copy of your actual one I believe that varies from county to county. Look in your phone book under local government and call the "Vital Statistics" dept. and ask them. I think I paid about $20.00.

As birth certificates are issued by the various states, you would need to contact the Department of Health in the respective state.

Depends. I paid $15 for mine a couple years ago and had to sent a registered letter requesting it. (Kansas)
