How did federal housing policies institutionalized segregation and wealth disparities?

Public housing project were used to house growing black population in some northern cities to prevent them from moving into white neighborhoods. The occupants were income limited so they became cluster for the poorest of the poor, often families with no adult male. One of the worst examples was the Taylor project in Chicago which at one time housed 27,000 people.

FHA loan were often not available in city neighborhoods and conventional loans required 20 to 30% down payments vs 5% down for suburban FHA loans.

The biggest decrease in black poverty happened during the liberal 60s.

That does tend to happen when you build big buildings and stuff them full of poor people. Surprise...you also fill the places with bums and druggies.
