Formally, it's Ph.D. which stands for philosophy doctorate (or more commonly phrased, doctor of philosophy).

Then again, those of us Ph.D.s who are not turgid will accept PhD or phd or phud or piled higher & deeper...

According to the AP Stylebook (2005):

Ph.D.

Harvard University writes "Secondary fields can greatly enhance a student's PhD program." at http://www.gsas.harvard.edu/current_students/current_students.php

It would not be 'phd' or 'PHD' since the 'Ph' represents the word Philosophy.
