a doctor stated "I believe that the root cause underlying the mistreatment and needless misery of the dying is that America, as a culture, has no positive vision and no sense of direction with regard to life's end."

what does he mean, can you discuss this statement of belief?

He believes that America, as a culture, thinks that dying is a BAD thing and not a GOOD thing.
In other words if you Americans looked at death in a positive way, as the end of a journey (life's end), there would be less misery about it.

I think he is saying that American doctors prolong life no matter how much pain it causes, without much regard for the quality of said life.
When hospice is called in to aid a patient at the end of life, we rarely see or hear from the doctor again. I don't think there is much training or education for the medical community on the process of dying and it's affect on all aspects of the patient's life. They could be such a support- and there are some that are- but not enough.
