MALL GIFT CERTIFICATE: Where is the code and pin?

I got a mall gift certificate for my birthday, it is green and is from www.mallgiftcertificates.net. I was trying to use is online to buy something from a store in the mall that it is for, but i dont know which numbers to put in for the code and pin!
