If you want to be scared from the beginning to end of a movie, I thought 13 ghosts was pretty good.
For one that builds suspense then has you wizzing on the floor, The Omen (the first one, not the remake).
Saw was a good disturbing movie too.
The Exorcist is good, and Exorcist III (uncut) will give you the willies in a dark room all alone.
Bordello of Blood - with Dennis Miller - a good vampire movie with a twist (a guy movie with rampant female nudity, sex, gore and violence).

The Exorcist was just recently voted as the scariest horror movie of all time. You should see the directors cut if you are going to bother seeing it at all.....

Carrie is one my favorites even though it's not really scary.
Dressed to Kill has great Angie Dickinson scenes at the beginning.
Body Double is a stupid movie but you should watch it anyway.
