If you publish only the titles, you're OK. If you publish excerpts from the lyrics, even fairly short excerpts, you need permission and no commercial publisher will touch the book without permision from the copyright holders.

No, you don't, unless you are passing them off as your own creation (ie printing the lyrics in your book and pretending that they are your original poetry). Copyright law, generally, only applies to imitation with the same medium. However, it is nice to acknowledge the writers/performers of the songs you're mentioning. The often appears on the copyright page or an additional flypaper.
