A really wild guess here...no research...and by the way, even if our medicine is more sophisticated sometimes now than it used to be, it doesn't mean "old wives tales" don't work.
Chemistry hasn't changed...and gross as we think some things are...sometimes they work. Chemistry tells me that...Ammonia cuts the wax buildup on a kitchen floor?
It may be that it was applied topically as an earwash. Since urine contains ammonia, maybe it is able to break up and flush out ear wax, which, (in overabundance) can clog the ears and cause infection to back up behind the "plug".
I play a really good game of Balderdash by the way...when something stands to reason, I can usually see it and sell it but it may not be actually true. Not trying to fool anyone but my speculation is as good as anyone else's.

In the "old days" we had less knowledge and more superstition. My mother died recently but she had some of the wildest notions of anyone I ever met. She was born and raised in a little railroad town in West Virginia. There are still people in that town that create wives tales, not to mention practicing them and passing on the silliness.
Some of the characters portrayed on the Andy Griffith show are far more accurate than a lot of people would like to believe.
