Ok, if someone had a BA in History from Georgetown and an MS in Mechanical Engineering from MIT, would that person have had to double major in History and Mech. Engineering while at Georgetown?

Very often, the top graduate schools will put very much weight on what you've done over what degree you hold. So, if he had the BA in History and some engineering courses, but he was also an avid builder of very high tech robots -- they'd look at his work in the field very favorably.

Very many people have an undergraduate degree in one major but also have enough classes in other subject to have majored in that as well without declaring the major.

There are some masters degrees you can get without having majored in it in college, but engineering and the sciences are not among these fields. Yes, you will have to take the necessary undergraduate courses to get into an engineering masters program. Especially a good one like MIT's.
