It depends upon the jurisdiction. First, of course, there will be several recounts with every ballot being closely examined and questioned. If at the end of the recount, it is still tied the options tend to be as follows: 1) a new election; 2) breaking the tie by lot.

As noted by other answers, for president of the U.S., the Presidency is decided by electoral votes. If there was a tie in a given state, given the quick time frame, you would probably get an emergency special session of the state legislature to allocate the electoral votes (probably something like giving each candidate one electoral vote for the state-wide tie than allocating the remainder based on congressional district). If there is not an absolute majority of electoral votes at the national election, Congress decides between the top four candidates.

It depends on the jurisdiction. You hear about this kind of thing a lot. In a really small town in Nevada a few years ago, there was an election where the results were, like, 18 to 18. The city charter said that the two candidates had to draw cards for the winner. The highest card became mayor!

Sometimes they do random stuff like that, but I think most times some additional government body gets involved, like the city council or the state legislature or a board of judges.

If your referring to the Presidential election, it has nothing to do with total number of votes cast.

Candidates win electoral votes for each state they win. The candidate that receives 270 electoral votes wins.

You can do an internet search to see how many electoral votes are allocated to each state and get more information on the electoral college.
