If he likes racing take him to F1 in Braintree, lots of fun.
Take him to Maine for seafood.

Some questions regarding my possible idea...any kids? If so, can you get them out of the house for a night or two? Is he birthday during the week or on the weekend? So, with those in mind...

Step One: Meet him after work (at home) with nothing on but a black ribbon around your neck...and of course his favorite drink in your hand...walk back to your bedroom...believe me, as a man, I know he will follow, and let nature take it's course.

Step Two: Dinner at a great dive...if you live in Boston, you know you can get great seafood without going to Maine. Pick a dump where you can grab a lobster (if he likes them) or whatever, has extremely cold beer and TVs to catch a game...or, have dinner early enough to catch a Celtics or a Bruins game after dinner...or even that movie he's been wanting to see (even if you don't as it's not your kind of show) and do pick his kind of joint to eat at no matter what his favorite food is...even if it's a burger.

Step Three: Repeat Step One when you get back home. Remember, this is a guy's birthday...we're from Mars, you're from Venus and we do think about it every 45 seconds!

Get him talking about his favorite birthday and if you can try and recreate it. I mean if his best birthday was going to a sports game, buy tickets to a game. If it is just being at home with you, fix his favorite dinner and rent a couple of his favorite movies.
