One other addition to the earlier (quite good) answer - the nutrients (nitrogen, etc.) are dissolved in the water and absorbed ("taken up") through the roots.

Hi Kristen, This sounds like a homework question to me!!! I'll be glad to help you.
N, P, and K refer to the three main nutrients plants need: NITROGEN for green growth and in compost piles to speed decomposition; PHOSPHORUS for energy, root growth, disease resistance, and production of good fruits, vegetables, flowers and seeds; and POTASH for strong stems, vigorous roots and increased disease resistance
There are a various natural sources for all of the above and I've included a link to a site that gives some good basic info on each.
As to the rest of your question of what else a plant needs to exist there are a number of trace elements that are required by different types of plants ie. iron, copper magnesium....
Different plants also need different degrees of sunlight and warmth to be happy and flourish. It all depends on the type of plant.
All plants require carbon dioxide gas. They absorb this through their leaves. Through a process called photosynthesis the carbon is absorbed as food and the dioxide is released as oxygen.
All plants require water to survive.
