It would have to be when I was a kid riding my bike. I was moving along at a pretty good clip, but wasn't looking where I was going and ran into a 3-ft. high concrete wall.
I flew forward and the bolt in the middle of the handlebar caught me directly in the crotch. I was doubled up on the ground for a good ten minutes before I was able to even think about getting up. Not much more needs to be said.

One of the worst physical pains I ever felt was when I tore my rotator cuff and separated it from the bone. That stuff hurt! The surgery was just as painful (afterward).

When I dislocated my knee....but it wasnt so much pain as the world just going upside down. I went loopy w/endorphins, the world was just messed up, and it hurt like "OH ****" but somehow it wasn't exactly pain, it was like the worse pain I've ever felt but numbed down to the point where it hurt REAL bad, but at the same time it wasnt THAT bad.
Weird as hell.

physically...probably when I stepped on a nail, but the oddest thing is that i don't remember it hurting, although i'm sure it hurt like mad.
breaking my wrist really hurt, but I was outside in the middle of winter in Canada and i took my glove off so my wrist got really cold and kind of numb, so that relieved the pain and it didn't actually hurt too much after that
