Well, a lot of Doctors do not have English as their first language. That can mean both their spelling is poor, and another thing- you can't understand a word they are saying to you. Which personally, i think it quite ridiculous.
Also, many Doctors have terrible handwriting- so maybe misread handwriting is the problem, not spelling. You would be amazed by the correlation between intelligence and poor hand writing.

With the test that I just got he wrote out kind of what he had found. I could not make out one word that was written so I asked a nurse what it meant and she looked at every way to try to explain his report to me and she couldn't even read his handwriting so she went herself and found that doctor and asked him to please put it in English and not gibberish. Well, she finally came back and she told me what it said and it was nothing bad. I think they just put it mambo jumbo and let someone else figure it out.
I think they don't take the time to write it out properly as they have another patient waiting that is only considered as money laying on that table.

those are very funny and I am sure that is how they were written up. However I agree with slk they dont write they talk into recorders and someone else has the job of writing it down. But it could be thats the way they said it if they were distracted or interrupted while doing so.

I'm impressed that you could actually read the doctor's writing to give us some amusement, but your question relates to syntax, not spelling.
