They dont get them because of unhappiness. They get them if you are a little lax on their hygeine. If theyre hygeine dips for a certain period of time, they'll get zits. You can rememdy this by having your teen sim click on a mirror and apply cream (or something like that) should be an option.

I have no idea. All I know is that when you Shift+Click a teenage Sim, and click Toggle Zits, they get zits! xD

They get them when they are not happy. Their levels need to be up, especially for fun and environment. Keep an eye on their wants too. That is very important.
