It is gramatically incorrect to use them both togethr, technically it should just be a question mark as it is a question. However, the common practice of using them together, people generally put the question mark before the exclamation.

They're two entirely different things, and you don't use them together - ever!. At the end of a question you would normally use the question mark > ? < and if you are exclaiming surprise of any type you would us > ! <
For "effect" you can always use two or more of the same signs together although this is frowned upon by the purists ! ! !

If you are just writing to your friends, it doesn't matter. But if you are writing for a class or publication, or something as that, use only one or the other. Technically, if you just use "How did you get that!" it is still understood that it is a 'question' but that it is also exclamatory.
