...for just general, never-get-tired-of-watching, 'just gotta watch it at least every four to six months' type of viewing, it's pretty much neck & neck, between "Smokey and the Bandit", "Superman: The Movie", "Better Off Dead", "Army of Darkness" and "Reservoir Dogs" (...I've lost track on how many times I've seen them, in any of the mediums you mentioned)...
...however, for something much more pin-point ritualistic, we're definitely talking "The Rocky Horror Picture Show", with an overall theatrical viewing count of 168 times (...video viewings don't count, in experiencing this film!!!)...

Star Wars. I can't count the number of times I've seen that one. When it was in the theaters I think I saw it 8 times (one day we sat through it twice) I have both the vhs tape and dvd, and have watched both of them many many times.
