Should you call the doctor immediately or wait a while? What is the earliest they will do an ultrasound?

And what usually happens at the first appointment?

Definitely call your Dr., however some won't want to see you until your 6, or even 8, weeks. An ultrasound depends on the Dr. and whether they suspect a problem or need to 'date' the pregnancy. I had an ultrasound at 7 weeks with my first due to bleeding, and again at 26 weeks. With my second, I had zero ultrasounds because the Dr. did not do them without a 'reason'.

At the first appointment you'll likely have blood drawn, have a chance to ask questions, maybe an internal exam (I had a pap done), and get your prescription for prenatals. It's been awhile, I could be missing something.

I called and set the appointment up, but they wait until about 8 weeks to see you and do the first ultrasound. They will do a pap smear, and order all your blood work. You should already be taking prenatal vitamins now, but most docs will offer a prescription for them if the ones you take are bothering you at all. They take a urine sample, and get your blood pressure, and weigh you, which will happen at all your future appointments too.
