She is severely dehydrated. Her eyes are still there, only sunk in. It may be beyond help.
First, try to get a poop sample to the vet, to rule out pinworms.

In the meantime, mix a slurry of applesauce, pedialite, butternut squash, milk thistle, and Ensure (not chocolate). Feed it to her by placing a drop on her nose. You may have to force-feed her with a syringe if she is that bad. Let the vet teach you how.

Give her a warm soak, and place a moist hide in her tank. It will help *slightly* in her rehydration, but will not solve the problem without professional intervention.

Good luck

i would get her off sand or any loose substrate she is on and put her on paper towels, reptic carpet, or newspapers...permanetly. Then I would try soaking her in warm water while giving her a belly rub. This should help her poop out anything that might be stuck and hydrate her. Then i would take her to a vet to see if she is impacted.

Could be many problems. Are you using sand? She could be impacted. What are her temps? What is her set up like? She could have an eye infection or stuck shed skin in them. You need to find an experienced herp vet asap.
