Ring your school and ask to speak to the exams officer. He may be able to tell you which exam board you sat and if you are very lucky he may be able to give you your results free on headed note paper.

If not you will have to contact the relevant exam board.
However,if you have not claimed your certificates or just lost them they will only give you a statement of results

"Statement of Results for a Candidate
AQA can issue a statement of results to a candidate for personal reference. This is an official document which can be used in the same way as a certificate. It is universally acknowledged as proof of the examination results which a candidate has attained and can be used in place of the original certificate. There is a charge of £34.50 per examination session, per level and per board for the Statement of Results for a Candidate"

I'm surprised you don't know them.
As suggested, you could approach the relevant examination board, but I suggest that you contact the school as your first move.

apply to the relevant examination board ( this could be a different board for each subject)
