Too many to list, but let's start with dirty seats. Can't they bother to wipe the seats and chairs when they're wiping off the table?
I hate going to those buffet style places where invariably someone thinks it's okay to let their kid practice filling his own plate. GAG. They do the touch and taste thing. Gross.
Dirty silverware, plastic drinking cups, not enough napkins, dirty salt and pepper shakers, children allowed to run around because their parents are too busy visiting, children that are allowed to stand up on the seat and turn around and bother the occupants behind them.
Gosh I sound like an old fart.
This was a can of worms.

when the waiter or waitress forgets you and you sit there forever waiting to put your order in or get the bill! i'll bloody walk out of places like that without paying if i've asked for something more than once and fifteen minutes later i'm still waiting for it.

When the waiter/waitress doesn't remember who ordered what. I don't care to hear that "but they're busy" crap either. It's their job.

Absolutly hate it when i order lemons in a water or for a meal and they leave the seeds in. Arg!!!!!!!!

I can't stand it when someone who arrived and ordered after me gets their food first. I'm very impatient when I'm hungry.
