You don't. To be honest, sometimes I would rather see the understudy. Stars can be overrated.

If you really want to know, you'd have to go through various methods to find out if there is a regular schedule for ther understudy. Sometimes there are 2-4 performances a week that an understudy will go on in place of the lead actor. You can e-mail or message their official websites and/or MySpace pages and see if you get a response. You can go to the theatre or try calling and see if anyone will tell you. You can go to the stage door after a show and see if the person you want to see will just tell you what their schedule is. Tell them you're going to buy tickets but you really want to see them perform the role and you're sure the understudy is wonderful, but you really would prefer to come on an evening (or matinee) when they are performing. Flattery will get you everywhere. If they regularly skip Thursday evenings and Saturday matinees, they'll likely tell you.

Of course, in the event of injury or illness there is no warning and you will still see a great performance.

I would suggest that you ring the box office of the production you want to see, find out if all performers do all performances, or if not, which ones they don't do (i.e. matinees). You can then decide which performance ideally you would like to see. But if there is a last minute injury or illness, then the staff may only know on the day of performance. Sometimes, the understudy can be just as good as the usual leading man or lady.

There is no way to know. The box office may not know until a few hours before curtain, if then. Don't be put out if you see an understudy. They might actually be better than the "star" you want to see.

You can't know for sure. Sometimes the universe intervenes and causes an unavoidable illness or emergency. Tickets are generally non-refundable.

The only exception I can think of is when a star goes on vacation or break to do another project- you may be able to find out this information through broadway news websites. Watch out for shows that bring in a star for a limited engagement. Sometimes the replacement may be another star, or maybe not.
