If you have amniocentesis, it's around 16 or 18 weeks (whenever they do that test now). As far as an ultrasound--I have had soooo many friends told they were having a girl/boy from their ultrasound...and they delivered the opposite gender. Of course, this is AFTER they had decorated a room, bought clothes, etc.
The amnio is foolproof, because they look at chromosomes. Ultrasound is not foolproof, it depends on the tech performing it, how the baby is positioned, etc. Even then, they can read it wrong.

For an ultrasound the best time is about 20 weeks. They can also find out the childs gender by the amniocentesis test, but not everyone has that done it's usually done around 14 weeks if you do get it done.

Usually around 16 weeks. The later the better and more accurate they are at guessing though. I found out I was having a girl at 21 weeks.

Between 16-20 weeks are the norm for when docs check. It's better to check closer to 20 for the organs have developed more. Some docs like to do it between 20-22 weeks but the further and bigger you and baby get it could possibly get harder to see the genitals. I got both mine at 18 weeks.

I found out the gender at my 20 week ultrasound. You pretty much have to wait 20 weeks to be sure.
