Batter is out and if there are runners on base, they can advance at their own risk. The ball is in play until it is thrown in. The play doesn't stop until that occurs. A teammate can retrieve the ball and throw it in. This happens more often than you think. Also, happens when an outfielder crashes into the wall making a catch.

Can you clarify? What do you mean what is the ruling? If the outfielder that caught the ball caught it on a fly, the batter is out. Otherwise, its up to the outfielders to get the ball back infield ASAP before the runner runs the bases.

Batter's out. Runners can advance at will, tagging after the catch. Another player on the defense will need to retrieve the ball unless the umpires call time due to severity of injury.

This actually happened in 1969 in Candlestick Park. Willie mays is in center and Bobby Bonds in right. The batter hits a long fly to the rightcenter fence. Both Mays and Bonds collide as Willie reaches over the fence to catch the ball. As they both tumble to the ground, Willie has the breath knocked out of him by Bond's knee. Willie layed on the ground as Bonds reached into his glove to show the umps the ball was caught. Best catch I ever saw on TV(was the game of the week).

There is no ruling; play continues. It's tough luck for the defense. The same thing goes if a fielder gets his foot caught in the fence or something.
