You really can't fairly make an assesment of this type.
The reason can and is multidimensional in this respect.
What software is being used. If the developer is starting from scratch, and if they are writing all of it with code, or are they using already developed clip art and backround pages, that are readily available. The graphics can easily be added and any links, buttons, Store tabs, same as well.
I have developed and published a site from scratch in 2-14hr days.
I have also spent 3 weeks doing one it really depends on a large number of factors, and how fancy you have it.

This is highly dependent upon your skill set and the tools available to you. With a decent content management system (Drupal, etc), depending on what the user form is supposed to do it's possible to have what you're describing in play in 30 minutes. This assumes that there is a pre-existing module that handles the type of input you're trying to solicit from your users, and that a default stock site template is acceptable.

At the other extreme, if your user input equates to an involved web application, and you're tasked with coming up with a truly custom look & feel for the site, it could take weeks to hash out the design, create the graphics, build the site templates & form(s), the cut a database schema and back-end code for the user input.

Without more detailed information on what this site is meant to accomplish and what level of customization is necessary it's impossible to give a good estimate.

Depending on how busy the designer/programmer(s) are, 1 or 2 weeks.

I have prepared mine in yahoo in 30 minutes. I wrote everything. The only missing thing is a photo I am just asking the support people where to find one suitable to my business.
