"Undefined" means the denominator is ZERO. Your example in 4-x / x-4 , at x=4, the denominator would be zero (so would the numerator, but that only means the numerator is zero).

"Domain" is the the range of x in which the function is NOT undefined. For instance, in x+3/4, this function has a value for all x, even if it is zero. Others have "holes" in them where the denominator is equal to zero. For instance, the quadratics in the denominator DO factor, so the roots will be points where the fraction or function is undefined.
