Personally, I'm not a fan of anything that resembles a cartoon character. So... Phoenix, San Jose , Florida "Panthers" and Nashville are on my worst list. The colours are fine I just can't stand something that looks as if a 5 year old would wear it as a t-shirt. Pittsburgh is alright though. Atlanta could stand to change their logo as well.
Canes have a really nice Jersey especially since it is relatively new. Boston, Montreal, New York "Rangers", Edmonton look the best.

I don't know about the best since I haven't seen all of the new ones. However I think Dallas has to have the worst. I love my home team but their new jerseys are horrible! The are only a little bit better than their own third jerseys. Now those have to be the worst jerseys in the history of sports. The old third jerseys always remind me of a woman anatomy. I would never were a jersey like that. No matter how much you paid me.

Current Best: Montreal Canadians
Best Vintage: Buffalo Sabres (blue and gold)
Current Worst: Toronto Maple Leafs (I know you don't want to mess too much with a classic look, but their new jerseys are just way too plain. They look like practice jerseys. Add some type of shoulder patch and I think they'd be great.)
Worst Vintage: Vancouver Canucks (The ones with the strange V striping from the 80's)

Best: Montreal Canadiens of 1944, the white one with a blue line across the chest with the C. I'm also very partial to the blue jersey for the Edmonton Oilers, but that's just because I'm a fan.
Worst: Philadelphia Flyers home jersey - the orange one. The black one is nice.
