I think its about the dysfunctional relationship of a drug addict.
"I hurt myself today"
"the needle tears a hole"
"I will make you hurt"
"You are someone else. I am still right here"
"If I could start again a million miles away. I would keep myself. I would find a way."

It's fairly obvious. The singer, at the end of his life, has had experiences with drug abuse and heart break and all the other human foibles. He calls life "this empire of dirt". He isn't sure that, given the chance, he'd do it all again.

It's kind of the anti "My Way".
