Psych evaluation: on record?

A friend and I want to settle a bet on who is crazier. We were figuring we could both take the same basic psych eval, but he was nervous about it going on some sort of record.

All I want to know is,
Does it generally go on some sort of record?
If so, is it possible for it NOT to?

No, and maybe while your there ask them to define crazy for you, or rather, ask them what the differece between being crazy, and having a "disorder" is, learn something...... because unless you guys are having halucinations or delisions, neither one of you is "technically" crazy.
