For civil cases -- true -- juries must be requested.

For minor criminal cases -- true -- if the sentence does not include jail time, the person is either not entitled to a jury trial at all or doesn't get one without asking -- in most states.

For major criminal cases -- false -- a jury trial is automatic in all criminal cases where jail time of over 6 months is possible, unless the defendant consents to a bench trial.
