If you really are trying and still having trouble, there could be a number of ways to help.
Ask to get your eyes checked, you may need vision correction. (Same for your hearing.)
Go for a check up and tell the doctor what you have said here. You may have a chemical or hormone imbalance that makes it hard for you to focus and study. Medication can help.
Do you have trouble reading? Talk to your teacher or a school counselor about getting tested for dyslexia--this is a disorder where letters and words seem to get scrambled on the page. A special tutor can help you over come this.
It may be that you never learned how to study efficiently, a tutor can help you with this as well.
Some people learn better by hearing information, some people learn better by reading, some by writing. A learning specialist can test you and help figure out what works best for you and teach you stragedies for improving your understanding of the material. That's really the goal, to learn the subjects.
This sounds like work, and it sounds expensive, but if you can overcome these problems your life will be better forever. Some health insurance will cover some of the costs. If not, your city or town is bound to have health clinics where you can get low cost healthcare. Your school or the school district will have resources for the tutors and learning specialists. You may need a parent, a sympathetic teacher or counselor to help be your advocate, but you need to fight to get the help you need.
If you want to improve your grades and are willing to put some effort into getting better, you will improve.

I guess only you know the best.
Find out what's the ROOT cause.
Could it be your learning style, attitude, exam fear or any other factos?
Once you have identified your ROOT cause, then you seek for the specific solution to your issue.
If you have challenges with finding out the ROOT cause, speak to someone you feel comfortable with, that person could help you realize something about your learning and studying styles. :)

5 totally understand because i had toughest time last yr. w/ math. but my problem was that i didn't study..maybe u should get a tutor and keep working really hard. see if your teacher will let u make up the quizzes but you wont get full credit.

cope up with your lessons double time!
give some extra effort for your grades: do group study; get a tutor; study whenever you have a free time; familiarize or master, don't memorize!
