I'd go for the 1999 European GP Ferrari pitstop for Irvine. 43 seconds it took because the mechanics had to frantically search for a tyre. This huge delay allowed Mika to catch up and finally overtake Irvine and gain 2 points from that race. Irvine lost the championship by two points!
Just out of interest, the Ferrari mechanic knocked down by Schumi was Nigel Stepney.

Yeah, that was the mid 90s.......1994 if I remeber rightly as that was Jos "The Boss" Verstappen! Damn, he had a great following :o)
Anyway, yeah, that was really bad though I would expect a lot of the older ones were a lot worse. Have you see any videos of really old pitstops? They were terrifying. No special fuel caps or fuel rigs, just cans full with fuel. Some one would manually remove a cap off the petrol tank, stick a funnel in the hole and pour petrol in. They knew when the tank was full as petrol kind of poured out everywhere. Plus there wasn't really a pit wall, just a...well, white line to seperate the pit from the track. I would imagine that people have died in the past.
Also, away from F1, indy car pitstops can be really bad as they don't use petrol like in F1 they use methanol and when it catches fire the flames are really difficult to see.
anyway, I have some videos so have a look.
IRL pit fire...stick with this as it's not really obvious that there is a fire...well, the blokes pouring water on themselves is a giveaway of sorts.

and a pit stop....1950s style!

I've just noticed after reading some other answes that I have found exactly the same video as Dr Quills...haha...sorry dude, didn't mean to copy, I just kinda got into this one. Have a thumbs up. :o)

Many of the other incidents mentioned above are good examples, but coming into the pits, there is always a risk of fire or running down the crew.
The worst one i have seen is probably by the toyota team sometime this year. I think it was ralf who came in to an empty pit and had to wait a few extra seconds for his crew to set up and sort themselves out.

Yeah this was bad but there have been more accidents during pitstops. Remember in Canada 2004 when Nick Heildfeild almost drove over one of his jordan mechanics?

I sorry but I think this is the worst I have ever seen I was watching this back when NASCAR was cool. Very sad day. Most of you probably were not born yet or knew what racing was.
They had ne speed limit back then on pit road and Rudd came in too hot and lost control and basically smashed Mike Rich.
This is why today there is a speed limit

Most of the bad ones have already been covered on here. So i am going to say this one despite the fact David Coulthard doesent actually manage to make the pits.but i am choosing anyway, just because Murray Walker is a legend

Yeah you are referring to the benetton fire in the mid nineties. This accident caused a major rule change in formula 1 hence the pit crew space suits. My personal favourite is the schumacher incident when he clipped a trolley and broke his mechanics leg before blaming him for standing there[2001 i think]. The best view is the on board camera facing backwards on the dvd.. I watched it over and over..
Fuel spills in indycar can be[debatable] even more entertaining as the methanol fuel burns invisible..

At least that can realistically be called an accident, the Nakajima thing at the weekend was stupidity pure and simple.
