I don't know if you consider her a action star or not.
My guess is Pearl White. She stared in the "Perils of Pauline". It was a 1914 silent cliffhanger serial shown in weekly installments.

Probably Rachel Welsh in "10 Million Years BC" and "Fantastic Voyage".
She was such a versatile actress. She looked great in torn animal skins or in a wet suit.
