Officers who wear stars (admirals and generals) are usually not in the position to be in direct combat.

In his career, Patton was on the Pancho Via expedition and fought in WWI. Those events would be where he was in direct combat.
The key phrase to your question is WW2.
As a Lieutenant General, then later four- star General, he was not directly fighting the enemy physically. I've read his bio; to the best of my knowledge, he never fired the revolvers in World War II DURING COMBAT. Still, he had to be qualified for his chosen weapons, and he certainly didn't wear them for show. Well, maybe he did, but not unless they were loaded and operational.

While most chose .22 revolvers, Patton felt the event's military roots garnered a more appropriate weapon, the .38. During the competition Patton was docked for missing the target, though he contended the lost bullet had simply passed through a large opening created by previous rounds from the .38, which left considerably larger holes.

Yes, he periodically used all the sidearms in his collection - maybe not in combat - but most certainly for target shooting and practice.
