You can speak fluent English now while sitting before your monitor screen thanks to "English Town". At first, you take a free online test to determine your English level then you will receive upon free subscription a daily email of issues taught during the day. However, and since this may not be enough for you, you can register (this is a payable service) to attend classes with others. You will be connected with other teachers and students with whom you can chat and improve your language. Go and see for yourself : www.englishtown.com
And if this is not to your taste then you can do what I do: 1) read English newspapers on a daily basis (The Guardian, the Independent etc.); 2) translate articles from your native language into English. If it worked for me then it should work for you too.

Talk to English speaking people! What is your first language? If it is a fairly commonly taught language in schools you could work as an assistant. I go to a language college and learn French and Italian. We get some great assistants who help the classes and this seems to be a good way to improve your language skills. Also reading magazines is useful. Finally try watching DVDs in English, with subtitle if neccessary.

Practice and insist that people speak only English to you and that they correct your English when you make an error. Watch TV and listen to the radio in English. Read books, newspapers and magazines in English as well.

You could try an English immersion school (like we have French immersion schools in Canada).
Or move to the UK for an extended period of time.
