They test in lots of different areas first, not just SF. Just like movies test in front of audiences before the final cut that you see when it's in wide release, they want to get opinions and get all the kinks out before the show officially opens and is available for critical review.

Not all shows go to San Fran. The Drowsy Chaperone started in Toronto if I'm not mistaken.
It's like a really big dress rehersal, they try it out in front of an audence. See what works, what doesn't. In some cases whole songs are rewriten, For example Legally Blonde the song Love and War was replaced with Postive.
Some cast members are changed. For example between Wicked pre-broadway run and coming to broadway three cast members were changed. Robert Morse was replaced by Joel Gray as The Wizard,
Kirk McDonald was replaced by Christopher Fitzgerald as Boq, and
John Horton was replaced by William Youmans as Doctor Dillamond
After the show is perfected it's moved to Broadway. Sometimes the shows doesn't make it past the pre run and is shelved.

you don't give a newborn steak so you wouldn't put a new fresh musical on Broadway. Like Wicked. the producers of Wicked weren't sure if audiences would take to it so it started in San Francisco and when the producers saw that it was overwhelmingly accepted then they moved to Broadway. if it wasn't accepted then they could just work on it there in San Francisco until it's ready for Broadway.
