If you enable Speech, it asks that you train for speech input, but you can skip this and highlight and click for Text to Speech.
There is a program called Navigator which is started with Start / All Programs / Accessories / Accessibility / Navigator
it reads the whole window or the letters you type - I have it on now.
If you need more elaborate control, then screen reader programs for the blind are available which include JAWS, about $800; Window Eyes being its main competitor. These give tuning controls.

If you have Office 2003 all you need to do is go into Tools and Click speech, a language toolbar will show up and it will have a command called Speak Text. All you need to do is select the text you want to read and click Speak Text.
