I doubt that they can phyiscally tell but it is very important that you inform your doctor if you are sexually active or not. if you are you will need to start having paps to detect cancer cells early. It is VERY important that you inform your doctor about your history, medical and sexually, in order for them to do their job and keep you healthy

If you Hyman is not there or has been broken that can be a sign of sexual activity. However, these can be broken by things other than sex.

If you are sexually active - don't lie to the doctor. If your parents are there - ask them to leave the room so you can talk privately to the Dr - tell them you are not a child anymore and don't need them in the room.

Then have an open an honest conversation with your Dr about your sexual activity and history. You should also ask about birth control. Since you are sexually active, you will need to have a Pap Smear and Pelvic exam once a year. Your Dr is not there to judge you - trust me, you won't be the first young person whose told them they are sexually active.

If it turns out that you have to let your parent(s) know about your being sexually active - ask your Dr if it is ok to bring your Mom (or which ever parent you think is most understanding) back into the room and your Dr discuss with you and your Mom about your choice to be sexually active and get on birth control.

If you think you are adult enough to be sexually active - then be adult enough to take care of yourself and your body. Get on birth control and start having your yearly gynecological exams. It's also a good idea to make getting tested for STD's a part of your yearly exams (even if you don't have any symptoms or have any reason to believe you caught something.)

If the doctor gives you a vaginal exam then yes. Don't worry. He is not permitted to tell anyone. It falls under doctor/patient confidentiality.
