I am not aware of any existing scales such as you mentioned. You may need to do some preliminary work with focus groups or "qualitative" research to get a feel for how to word these questionnaires. For example, speak with a focus group of volunteers, and get a discussion going that will help you to develop valid questions. Then, you have to validate your scales statistically. (I am assuming you are in a major research institution and have the assistance of statisticians who can help you to validate the questionnaire statistically) IF you do not have a good group of valid questions, you will have an instrument that proves nothing, no matter how internally valid it may seem.
Focus group: Have it be mixed gender, and ask open ended questions about blame/myths/attitudes. Be very non-judgmental as you ask people to describe their attitudes. Well, now that I am thinking about it, you may want to do more private questioning because people might get all mad and start to argue. Just let people talk, take complete notes, preferably recorded. From those notes, you can begin to develop questions. Then, bring in a new group of volunteers, and pilot the questions. See if they understand them, ;and if they answer them in a way that is easy to spread statistically. I guess you are familiar with a Likert scale, I would probably go for a 9 point Likert scale here, it will make shades of difference easier to spot.
Do you have a faculty advisor who has questionnaire statistics experience? If not, please get one, this is way too much to answer on YA!!!

Are you asking us to answer the questions or come up with more questions, I think the later.
What the perpetrator thinks of the victim.
What was the vicitm doing before the rape
What was the victim wearing before the rape.
